Why do some men struggle to reach orgasm with a partner? Research on delayed ejaculation, masturbation style and Freud's insight on inhibition.
زمان مطالعه: 8 دقیقه

Delayed ejaculation describes a man who has adequate erection and sexual desire, yet with a partner, despite sufficient stimulation, reaches ejaculation and orgasm only after a long time or not at all, while the same man often reaches orgasm during masturbation without difficulty. It is less recognized than premature ejaculation, but no less troubling for the couple. Scientific reviews regard it as multifactorial: the body, masturbation habits, anxiety, and the psychological meaning of the relationship all contribute. The good news is that many contributing factors can be identified and worked on.

What Exactly Is Delayed Ejaculation, and How Does It Differ From Simply Lasting Long?

In a review in Fertility and Sterility, Jenkins and Mulhall define delayed orgasm as a “persistent or recurrent difficulty, delay in, or absence of attaining orgasm after sufficient sexual stimulation” that causes distress to the person. Two keywords matter: “sufficient stimulation” and “distress.” A man who ejaculates late, and who is satisfied with that timing as is his partner, does not have a disorder. The problem emerges when prolonged penetration turns into fatigue, pain, frustration, or a sense of failure.

To understand what “late” means, we need to know what is typical. Waldinger and colleagues used a stopwatch to record the time from penetration to ejaculation in 500 couples from five countries (the Netherlands, the United Kingdom, Spain, Turkey, and the United States). The median was 5.4 minutes, with a wide range from about half a minute to 44 minutes. The median also fell with age, from 6.5 minutes at ages 18 to 30 to 4.3 minutes above age 51. The normal spectrum is therefore broad, and delayed ejaculation is diagnosed not by a stopwatch but by the clinical history and the couple’s distress.

Why Do Some Men Reach Orgasm With Masturbation but Not With a Partner?

One of the clearest clues is masturbation style. Michael Perelman, of the Human Sexuality Program at Weill Cornell Medical Center in New York, writes that many men with delayed ejaculation have an “idiosyncratic masturbatory style”: a way of stimulating themselves that a partner’s hand, mouth, or vagina cannot easily reproduce. In his view, delayed ejaculation is exacerbated by an inadequate combination of “friction and fantasy”: physical stimulation that does not match the body’s familiar pattern, and negative thoughts that neutralize sexual fantasy.

A clinical study in China reinforces this picture. Xia and colleagues compared 24 men with primary delayed ejaculation with 24 healthy men of the same age. The affected group masturbated more frequently, and some used a particular style. They reported fewer nocturnal emissions, had longer penetration-to-ejaculation times, and scored higher on anxiety and depression. Their penile shaft sensory threshold was higher. The researchers concluded that the body’s adaptation to a specific masturbation pattern may be related to delayed ejaculation. The study is small and cannot prove causation, but it is consistent with Perelman’s clinical observations.

Body, Anxiety, or Relationship: Which Is the Main Cause?

Abdel-Hamid and Ali, in the World Journal of Men’s Health, describe delayed ejaculation as an uncommon and still poorly defined disorder. Its mechanism is “multifactorial, including both organic and psychosocial factors,” and its exact cause remains unknown. For this reason, a careful history is the key to diagnosis. Has the problem existed since the beginning of sexual life, or did it start later? Does it occur with every partner or only one? Does it also occur during masturbation?

On the organic side, Jenkins and Mulhall point to medications, reduced penile sensation, hormonal disorders, and excessive penile stimulation, so a medical evaluation is an inseparable part of assessment. On the psychological side, Xia and colleagues found higher anxiety and depression in affected men, and Perelman describes negative thoughts that neutralize erotic fantasy. He also stresses that pressure to conceive and anger, whether the partner’s or the man’s own, are important causal factors that often require individual and couple conversations.

What Does Freud Say About Inhibition in Sexual Function?

In Inhibitions, Symptoms and Anxiety (1926; SE vol. 20), Freud counts disturbances of male sexual function among the principal examples of inhibition. Among them he mentions the interruption of the sexual act before its natural end, that is, failure to reach ejaculation. In his view, inhibition is a restriction of an ego function so that it does not enter into conflict with the id or the superego and anxiety is not aroused. This insight remains useful in the consulting room: a body that does not reach orgasm is sometimes protecting its owner.

Earlier, in “On the Universal Tendency to Debasement in the Sphere of Love” (1912; SE vol. 11), Freud had explained that a complete sexual life requires the convergence of two currents, the affectionate and the sensual. When the two remain separate, a man may let go less with a woman he loves and respects, and, extending this insight to delayed ejaculation, we can understand why a man who reaches orgasm easily in his private fantasy holds back pleasure in his wife’s arms. This reading aligns with Perelman’s finding on the importance of fantasy alongside friction. It helps the therapist, instead of blaming the man, to explore with him the meaning of this holding back.

Frequently Asked Questions — Behckam.com

Does delayed ejaculation mean low sexual desire?

Not necessarily. Many men with delayed ejaculation have sufficient desire and erection and can even sustain prolonged penetration. The difficulty lies in the orgasm and ejaculation phase, not in desire. If desire has also declined, however, hormonal and medical evaluation is essential.

How many minutes of penetration without ejaculation counts as “late”?

There is no fixed number. In Waldinger’s five-country study, the median time from penetration to ejaculation was 5.4 minutes, and the range extended up to 44 minutes. The main diagnostic criterion is persistent delay accompanied by distress in the individual or the couple, not a fixed figure on a clock.

Can masturbation style really be a cause?

In some men, yes. Perelman regards a particular masturbatory style as an important factor, and Xia and colleagues likewise reported more frequent masturbation, and sometimes particular styles, in affected men. Gradually changing this pattern, under specialist supervision, is often part of the work.

When should one see a specialist?

When delay or failure to ejaculate has lasted several months, has led to anxiety, emotional distance, or difficulty conceiving, or began after starting a new medication. A combined medical and sex-therapy assessment gives the best picture of the causes.

Conclusion: A Holding-Back That Can Be Released

Delayed ejaculation is neither a sign of coldness nor of “not being a man.” Reviews describe it as multifactorial, involving the body, masturbation habits, anxiety, and the meaning of the relationship, and Freud’s insight helps us understand why the body sometimes holds pleasure back. When couples become curious instead of blaming, and, with a specialist’s help, rebuild the pattern of stimulation, fantasy, and emotional conversation, many men find the way back to shared orgasm.
