Adult ADHD often follows couples into the bedroom: more desire, less satisfaction, and a blame cycle that couples can learn to break.

“You never listen to me.” “You forgot again?” “Why do you interrupt me?” These sentences are heard in many homes, but in the life of couples where one partner has ADHD (attention deficit/hyperactivity disorder), they become the fixed daily script—and this script usually does not stay in the kitchen; it follows the couple into the bedroom.

This article is presented by Dr. Saeed Behckam, clinical psychologist and sex therapist practicing in Iran (Tehran) and Canada (Toronto, Richmond Hill & Vancouver), for the readers of Behckam.com.

How common is adult ADHD?

A global systematic review and meta-analysis adjusted for the world’s population structure in 2020 estimated the persistent adult ADHD prevalence at 2.58% and symptomatic prevalence at 6.76%; that is about 140 million and 366 million adults worldwide.

When desire is there but staying present is hard, the attention gap starts in everyday conversation.

Limitation: The meta-analysis is based on studies with heterogeneous diagnostic tools, and valid data from Iran contribute only a small share. [Needs further review: prevalence of adult ADHD in the general population of Iran — no valid population-based study was found in PubMed; available Iranian data are mostly from specific samples such as prisoners or hospitalized patients and cannot be generalized.]

Why do nearly half of adults with ADHD have sexual problems?

The most important finding is from a specialized adult ADHD clinic in the Netherlands: 136 outpatients were evaluated; 39% of men and 43% of women had symptoms of sexual dysfunction, and 17% of men and 5% of women reported another sexual disorder. Before the study, only one male patient had received such a diagnosis.

The first systematic review of these individuals’ sexual lives describes a contradictory pattern: compared with the general population, higher sexual desire and higher masturbation frequency, but lower sexual satisfaction and more dysfunction.

ADHD is at its core a disorder of executive functions, but inside the relationship this deficit translates into the language of love: forgetting a promise becomes “they do not care about me.” In the bedroom, mind wandering, sensory sensitivity, restlessness, and novelty seeking make up the same combination of higher desire and lower satisfaction.

Limitation: The Dutch study is single-center, without a concurrent control group and based on self-report; the systematic review also states that the number of studies is small and samples are small. These figures indicate a pattern, not a definitive rule.

Why does the parent-child pattern shut down sexual desire?

When one person manages the planning, bills, and appointments alone, the relationship slides into an unequal structure: that partner becomes the supervisor, and the other partner becomes someone who must be looked after. This parent-child pattern may be the most important killer of sexual desire in these couples.

The psychoanalytic approach carries this observation one layer deeper. A child who for years was called “distracted,” “lazy,” and “incompetent” brings two things into adulthood: a harsh superego and a self-image that is always inadequate. Such a person has already condemned themselves before their partner says anything; for this reason, the mildest criticism is heard as a repetition of the old verdict. Sigmund Freud, in his paper “On the Universal Tendency to Debasement in the Sphere of Love” (1912), explains that when the two currents of affection and sensuality are not united in one person, sexual desire withdraws from the very person we love. Where care and supervision rule, eroticism steps aside; desire requires two equal adults, not a parent and an erring child.

In a 33-year prospective follow-up, boys diagnosed with ADHD at about age 8 were measured at age 41 against a comparison group: 31% had divorced at least once compared with 12% of the comparison group — a ratio of about 2.6 times, and in absolute risk terms about 19 more cases per 100 people. A 2021 review also concludes that romantic relationships of adults with ADHD are on average shorter and more conflictual.

Limitation: The sample in that 33-year follow-up was only White men referred to a clinic; the results cannot be directly generalized to women or to all ethnic groups.

For Iranians in Iran and in migration, what is the way out of the blame cycle?

The first step is accurate seeing: specialist evaluation gives the individual and the couple a shared language. But this step faces two barriers in the Iranian family and in migration.

Psychologist consulting a client about how adult ADHD affects a marriage and sexual intimacy

In Iranian culture, an adult who is distracted is called “disorganized” or “irresponsible”; this moral language is amplified in the extended family and finds its way into the bedroom. Migration complicates the equation further: forms, credential equivalency, taxes, and children’s schooling in a language the person has not yet mastered create a heavy executive burden, and the partner often takes on the role of project manager; the anger that grows from this unequal division appears months later as low sexual desire.

The second step is returning the relationship to two adults: redistributing responsibilities based on actual abilities, entrusting memory to a shared calendar and written agreement, and asking “sign or intention?” before judging. In the sexual domain, planned intimacy is a form of showing affection for a mind that gets lost in time; reducing stimuli, talking about sensory preference, and accepting mind wandering move couples from a distracted relationship to a present relationship. Individual and couple work with a specialist shortens this path.

ADHD is an explanation, not an excuse; it does not remove responsibility, but it removes shame.

Frequently Asked Questions — Behckam.com

Does ADHD cause sexual problems?

Evidence shows a clear link. In a Dutch specialized clinic, 39% of men and 43% of women had symptoms of sexual dysfunction, and the systematic review reports higher desire but lower satisfaction. The cause is usually a combination of distractibility, restlessness, sensory sensitivity, and relationship tension.

Is adult ADHD linked to divorce?

ADHD alone is not a cause of divorce, but it is associated with higher divorce risk. In the 33-year follow-up, 31% of men with childhood ADHD had divorced at least once versus 12% of the comparison group. In my clinical experience, the blame cycle and parent-child pattern play a large role and can change with specialist support.

Why does my partner with ADHD forget what I say?

Forgetfulness in ADHD results from weakness in working memory and attention, not from lack of caring. The 2021 review describes these individuals’ relationships as on average shorter and more conflictual; entrusting memory to a shared calendar and written agreement reduces the pressure of this misinterpretation.

Do people with ADHD have higher sexual desire?

According to the first systematic review, these individuals report on average higher sexual desire and higher masturbation frequency, but lower sexual satisfaction than the general population. The number of included studies is limited, and generalization to all adults with ADHD should be cautious.

What do these findings mean in practice?

Adult ADHD was invisible for a long time, and what is invisible is usually judged in moral language. Today the evidence shows that this condition is tied to desire, sexual satisfaction, and relationship stability, but none of these links is fate. In the therapy room I have often seen couples who found the correct name for their pattern, came out of the blame cycle, and returned to a more equal and alive relationship. A mind that works differently can love deeply; it is enough for the relationship to become a place for two adults.
